Immunization Information


In accordance with Massachusetts State Law, it is REQUIRED that each student entering Newbury College have on file a completed Immunization Verification Form . Any student failing to comply MAY NOT BE PERMITTED TO REGISTER OR ATTEND CLASSES.

Immunization Requirements

Massachusetts General Law, Chapter 76, Sections 15, 15C and 15D, require all full-time students to provide documentation of immunization against:

Tetanus Diphtheria Pertussis

One TDap dose, if no record of DTaP/DTP/DT/TD within 10 years.

MMR (Measles, Mumps, Rubella)

Two MMR (measles, mumps, rubella) Vaccines
Dose 1 must be after the first birthday
Dose 2 must be at least one month after the 1st dose
Dates of serologic immunity for measles, mumps, and rubella.

Hepatitis B

Hepatitis B Vaccine Series (A total of three vaccines at varying intervals)


One Meningitis Vaccine within the past 5 years
Completion of a signed waiver (contact the Office of Residence Life for waiver information)

Vericella (Chicken Pox)

Two Varicella (Chicken Pox) Vaccine
History of Chicken Pox (health care provider diagnosed or date of serologic immunity)

Please make arrangements to obtain documentation of immunizations or receive required vaccinations immediately.

Please do not attach additional medical information (medical history, etc) to the Immunization Verification Form. Such additional forms should remain with the student. It is strongly encouraged that you keep a copy of the completed Immunization Verification Form for your personal records. Please note that it is Newbury College policy to not release medical or immunization records to either a second institution or to the student who initially submitted the documentation.
If you have questions regarding a waiver of these requirements for religious or medical reasons please contact the Office of Residence Life.


If you are interested in participating in College Athletics you MUST have a physical dated no earlier than three months prior to registration. A completed athlete physical examination form must be submitted to the Athletics department. The physical and immunization verification forms must be on file prior to any collegiate level athletic participation, practice, or game.
